Facebook page on blog?

OfflineI added a fan box on my blog. It worked nicely for a few days and now it is all grayed out. It used to show my last posts and encourage sign up as a fan. Any clues as to what might have gone wrong?

And a second problem...when it does show up it's a link to my personal page! I finally just went in and deleted it from the blog. There's a bug in it somewhere when the code shows it's going to the fan page but what shows up is my personal page.
