Form site for building different kinds of forms

I consider this a website tool as I used it to create a reservation form for my wine tours.

The basic form is free. The only thing you must remember is to go in when you receive your reservations in your email and remove the reservation as the free level will only hold something like 5 or 8 bookings and then it tells the person booking that the mailbox is full.

It takes a little getting used to figuring out how to create the form, but it sure beats trying to do it with HTML!

I much prefer to use html.   I've set up a reservations request form on my website utilizing a CGI script.   The advantage is that it is customized to fit my site and uses the same CSS file as the rest of my pages.

I used this to make my form alot easier than using html or anything else I have tried.  Then just access it from my web page.  This can also be copied to your page or so the FAQ says

You would still have to have a scripted call to a cgi or some other server-side file.
